Because foreign national interns/residents must be appointed on H-1 visas, the following information must also be included in the application packet:
A copy DVM diploma (plus original certified translation if not in English)
A copy of the front/back of I-94 form (if currently in U.S.)
List of employment in U.S. during past six years (dates, job title, name and address of employer) and visa status for each position.
Copies of all IAP-66 or DS-2019 forms (if ever on J-1 status in U.S.). If on J-1 visa, and you are subject to (but have not complied with two year home residence requirement), a waiver must be provided.
Copies of Form I-201D/EAD (if ever on F-1 status in U.S.)
Copies of Form I-171C or I-797 (if ever on H-1 status in U.S.).
House officers are required to communicate proficiently in written and spoken English with clients, colleagues and students and therefore, must have a sufficient command of the English language to perform this task without further language training. A telephone interview in English, conducted between the candidate and the relevant section chief or the chief's designee, will be used to determine the candidate's command of the English language and may affect the ranking of candidates.
