Accepting Your Awards

Once your financial aid offer has been prepared, you will receive an email notification and a Financial Aid Offer Packet will be mailed to your current mailing address on the Financial Aid Tracking page. The packet will include the following forms that will need to be completed and returned to our office:


Financial Aid Offer Letter/Acceptance Statement
(due within 15 days of offer date)

Verification Sheet (due within 15 days of offer date) form will be updated soon

Certification/Compliance Statement
(due within 15 days of offer date)

Verification of Enrollment for Siblings/Spouse (due by October 10)

 

First-time borrowers will also need to complete the following:

Loan Notes and Entrance Counseling Interview
Must be completed before the start of the fall semester to ensure loan disbursement for all first-time borrowers. Please click here for more information on how to complete the Loan Notes and Entrance Counseling Interview.

IRS Federal Tax Return Transcript (due by August 15)
Required of all first-time applicants for financial aid at the College of Veterinary Medicine (and randomly selected continuing students as notified) for each person whose financial information is included on the aid application. You can request the Tax Transcript from the Internal Revenue Service by calling 1-800-829-1040 or by completing the application form at http://www.irs.gov/efile/article/0,,id=232219,00.html after July 1.

 

NOTE:
In compliance with federal regulations, disbursement of financial aid is withheld until all required documents except Verifications of Enrollment of Siblings/Spouse (due by October 10) are received. Failure to provide all required documents by the stated deadlines will result in cancellation of any College grants or scholarships awarded.
