Application Forms Required For Review of Eligibility

The College Financial Aid Office will provide application materials upon request. If application materials are completed in a timely manner, financial aid offers will be made before the admissions deposit date.

Free Application for Federal Student Aid (FAFSA)
This application is available from high school guidance or college financial aid offices, or do the FAFSA on the Web: Home Page - U.S. Department of Education - FAFSA on the Web.

• Cannot file before January 1 of the year of admission.
• Used to determine eligibility for loans and work-study.
• Complete parental information is required on FAFSA application for students wishing to be considered for a 5% Health Professions Student Loan.
• The Cornell Title IV code is 002711

College Scholarship Service PROFILE
To register for an application call 1-800-788-6888 or connect to College Board Online on the Web: http://www.collegeboard.org.

• Used to determine eligibility for scholarships and grants.
• If you are less than 30 years old on January 1, of the year of application and wish to be considered for College grants and scholarships, complete the College Scholarship Service PROFILE and other applicable CSS forms, including complete parental data.
• The CSS Profile code for the College of Veterinary Medicine is 2822.

Federal Income Tax Returns
Submit signed and dated photocopies of student, spouse and parents' federal tax returns, including W2s and all supplemental schedules filed (Schedule A, Schedule, B, etc.).

• Note: Student's name and Social Security Number should be clearly printed on all forms.
• Note: Students who marry before October 1 of the academic year are required to submit complete financial information for their spouse.

Student Statement of Personal Interest
Two copies. (If applying for College grants and scholarships)
