The following items have summer deadlines.  Please ensure that you complete and submit any necessary forms to the appropriate contact person by the deadline. 




June 15 Health History Form

There are a number of steps involved in completing the Health History Form, and the process may take several weeks to complete. Plan ahead and read the requirements carefully.  Login to mycornellhealth with your net id to complete the forms.  More information can be found here:

July 9 Meet The Class of 2022 Survey

Click on the link to the left to complete the Meet the Class Survey and attached a photo.  The photo will be cropped into a small square.  You can submit any photo, but headshots work best. 

July 15 Cornell Identification (ID) Card request form due  (photo is needed) Students who meet this deadline can pick up their student ID’s from the Office of Student and Academic Services, S2009 Schurman Hall on August 9, 2018. If you do not pre-apply for a student ID, you will experience a delay in receiving your ID card and will need to go to Day Hall to pick it up after August 28. Student Health Plan Appeals Application

All graduate and professional students will be enrolled automatically in Cornell’s Student Health Plan (SHP).  The 2018-2019 SHP plan year will run from August 1, 2018-July 31, 2019.  Click here to see rates and details.  Only those students who demonstrate coverage by another insurance policy that meets Cornell’s requirements will be allowed to appeal the charge for the SHIP. The appeals application deadline is July 31, 2018. An appeals application will be available June 20, 2018.  September 30, 2018 is the deadline to choose to pay in monthly installments (with a small service charge). Commuter and Parking Service Form for Vehicle Permits

Beginning August 1, students can register their vehicle (required; free) and purchase a parking permit (optional; price varies) for their residential area or a commuter lot prior to their arrival. Introduction to Animal Use (Online)

August 1 CARE Training Registration Form

Please complete Section A1 and A2 of the CARE Training Registration Form.

For section A2, under “What species are you working with,” please write “VARIOUS” and check all the boxes directly beneath. You do not need to complete any other sections. If you need any help or have any other questions regarding this form, please contact Christine at cmp15 or David at dem23.

August 1 Emergency Contact and Photo Release All students must complete the Emergency Contact Form and Photo Release form. Please click each link and complete the form, once you have signed it, the form will automatically be sent to us.

Bursar Bill

Take care of all your financial obligations to the University by August 7. Fall tuition must either be paid in full from your own resources, or you must have accepted sufficient financial aid to cover tuition and fees, and promptly submitted all supporting documentation requested, so the estimated amount appears on the Bursar System.

Sign Online Master Promissory Note and Complete Entrance Counseling

William D. Ford Federal Direct Loans

For both Federal Direct Unsubsidized Loans and PLUS loans, go to
1.            Sign in using your FSA ID and password.
2.            Complete Entrance Counseling.
3.            Complete Direct Subsidized/ Unsubsidized and PLUS Loan Master Promissory Note (MPN).
Note: You must complete both the MPN and the Entrance Counseling.  Your loan(s) will not be disbursed until both are completed. 

Health Professions loans

You will receive an email from our student loan servicer Heartland ECSI with instructions for completing the promissory note and required disclosures. Make sure to check your SPAM folder to ensure that it has not been misdirected

Direct Deposit
If you are receiving financial aid and expect a refund for your living expenses, it is highly recommended that you sign up for Direct Deposit to expedite your refund. Fall refunds are issued in the last two weeks of August and if you are not signed up for Direct Deposit, you will need to pick up a check in the Bursar’s Office at 260 Day Hall.
