  • ALERT: Reagents for the ACTH assay are on backorder by the manufacturer. We anticipate our current supply to last until 8/21, after which ACTH testing will pause. Subsequent samples will be frozen until we can resume. The manufacturer's expected end date for the backorder is 10/8. We are actively pursuing possible solutions and will keep you informed.

Submission Forms

Please use dark ink to fill out submission forms. The forms are scanned into our system and the lighter/brighter colored ink fades and makes it impossible to read the form once it is scanned. Thank you.
